Adult social care supports the wellbeing and independence of adults aged 18+ with a diverse range of needs, including: those with a learning disability or physical disability, mental health condition, dementia, autism, and other long-term conditions.

If there comes a time when you need support from adult social care, our aim will be to support you to stay independent, safe and well so you can manage your own needs and achieve the life you want to live. We will do this by focusing on the things that you can do and ways we can help you to build your strengths to do the things that you need to do.

Short term care

Short term care covers a range of time-limited services designed to help you live independently at home for as long as possible. It’s often put in place to help you regain independence after an illness, fall or stay in hospital.

Short-term help could include:

  • help getting back to doing daily tasks independently
  • respite care
  • help to get around your home
  • advice on and provision of equipment

Short-term support is usually decreased, or comes to an end, as you become more independent.

Long term care

Long-term care is for complex and ongoing needs. It covers your personal, social and wellbeing needs. These could include:

  • personal help at home with washing and dressing
  • keeping you active and connected to your local community
  • support with employment or education

This type of care could be provided in a range of settings, such as in your home or in a care home.
