If you are 18 or over and need care and support, we are here to help. We also support young people moving from children’s services to adult services, including those who are neurodivergent.

How we can support

The adults care and support team works with people who have care needs because of physical disability, cognitive or functional difficulties, or conditions linked to ageing. We also support unpaid carers.

Our aim is to help you stay independent for as long as possible. We do this by focusing on your strengths and what matters to you. Support can include:

  • Short-term help such as reablement
  • Equipment and home adaptations
  • Telecare services

If you have eligible social care needs, we will carry out an assessment and agree a plan with you. Services may include:

  • Care at home
  • Residential care
  • Nursing care

Most services are provided by independent care organisations. All support is tailored to you and designed to help you achieve your goals.
