Why should you have a carers assessment?
A carers assessment focuses on:
- The care and support that you provide and how this can impact on your life
- Your goals and what support is available to help you achieve them
- What support is available to you in your local community, we can also help you to access this support
- A carer’s assessment can help you to understand what your strengths are
The carers assessment takes the form of a conversation in person or over the phone with a member of the Carers Centre team and it can be as long or as short as you need it to be.
If you would prefer to fill out the carers self-assessment form before speaking to member of staff, you can complete the carers self-assessment online.
Once we receive it, a member of the team will contact you about having a further conversation about your needs. This can be done face-to-face or over the phone on a day and time of your choice or via e-mail if you prefer. Carers assessment is not a requirement if you wish to access help and support from other organisation.
If you’re under 18 years of age please speak to the Young Carers Team at about support available for you.
