Do you have the experience to provide skilled support to maximise independence and to help the individuals remain living in their own home for longer. If so then this could be the role for you.
The Portsmouth Rehabilitation and Reablement Team is an integrated health and social care team, providing a short term Intermediate Care and rehabilitation service to vulnerable and disabled service users. The team consists of Rehabilitation and Reablement Assistants, Occupational Therapists, Physiotherapists, Nurses and Social Workers.
As part of your daily routine you willassist clients in a range of daily living tasks, for example with transfers, mobility, washing and dressing, toileting, household tasks, meal preparation, social activity and access to the community.
Be able to work flexibly between the hours of 7am and 10pm in order to meet the needs of the service. This includes working alternate weekends and have a full driving licence and your own transport as your role will involve visiting service users across the city on a daily basis within a specific time span.
