Salary - £36,000 p.a. Full Time Equivalent, pro rata to contracted hours.

Job type/hours - 18.75 working hours per week, worked over 4 or 5 days each week
This post is initially for a fixed term of 12 months.

This is a new post introduced by Hampshire Care, working alongside commissioners in Portsmouth from the Local Authority and CCG, which aims to support and facilitate discharges from hospital to local care and nursing homes.

Following a period of in-patient care, and prior to an individual's discharge to a Care Home, practitioners are required to undertake a statutory assessment or review to determine eligibility. Additionally, a separate assessment is required for individuals who are discharged into a Care Home setting in order to ensure full and complete information about the individual is collected in order to enable a safe admission to the Home.  

The purpose of this role is to support Homes, discharge teams and individuals (patients and their families) by carrying out assessments on behalf of providers of residential and nursing care.It is hoped that this will free up resources in providers, facilitate discharges from hospital more quickly and more smoothly and reduce post-admission problems in homes and readmissions to hospital.

The post-holder will need to develop relationships within and work alongside discharge teams at the hospital, but will report to providers and remain independent of those discharge teams.
