An exciting opportunity has arisen for a Higher Grade Occupational Therapist to join the Portsmouth Rehabilitation and Reablement Team (PRRT). PRRT is an integrated health and social care team, providing a short term Intermediate Care and rehabilitation service to vulnerable and disabled service users.  The team consists of Occupational Therapists, Physiotherapists, Nurses, Social Workers and Rehab Assistants. PRRT is at the forefront within Portsmouth providing urgent assessment in the community for hospital avoidance, rehabilitation and reablement at home, and facilitating a discharge to assess pathway from the acute hospital allowing patients to have their immediate needs assessed in their own home.

The post holder will provide skilled support to maximise independence and to help the individual remain living in their own home for longer. The Occupational Therapy service provides intervention to patients in their own homes.

PRRT is a diverse team with a very varied caseload. Due to the nature of the referrals, the clients present with varying needs from simple to complex and varying conditions across every speciality. A proportion of this role involves generic working as part of the assessment team which provides opportunity to experience challenging clinical decision making and to develop skills in assessment and management of un-well clients.

You need to have a Diploma of the College of Occupational Therapists or BSc Occupational Therapy or any Occupational Therapy qualification recognised by the World federation of Occupational Therapists and current registration with Health Care Professions Council.

You will be required to work a shift pattern on a rota basis, working over a timespan of 8am-8pm Monday to Sunday, including one weekend in four.
