We are looking for a professional to join a fully integrated Health and Social Care Learning Disability Team which is part of Adult Social Care, Portsmouth City Council.  We offer a city wide health and social care service to adults with learning disabilities and their carers.  We operate a named worker system which supports an asset based, outcome focussed, aspirational approach. 

This role is critical to the implementation of the Learning Disability Housing and Support Strategy. Despite a significant shift to Supported Living much remains to be done in terms of design and development of a range of innovative provision and at the same time ensuring that culture and practice actively promote independence and a sense of ownership.  We are looking to change culture and practice and will promote a model of Active Support. You will be employed to work with support providers to ensure they are delivering effective skills development programmes to their tenants, setting clear goals and achieving meaningful outcomes. You will be working directly with service users as well as educating and supporting staff teams to implement independent living skills programmes and facilitating a culture of aspiration.

You must be a qualified Occupational Therapist who has good assessment skills, good communication skills and is able to cope with workload demands efficiently and effectively. There is significant element of OT within the team and clinical supervision will be provided by Band 7 (Agenda for Change) Occupational Therapist.
