The Independence and Wellbeing team provides a preventative, community-based service that promotes independence, reduces social isolation, and enhances wellbeing. The team delivers early intervention through a range of inclusive physical and social activities, aiming to avoid, delay, or reduce the need for statutory care and support. This is achieved in close collaboration with local communities and voluntary sector partners, ensuring services are responsive, accessible, and rooted in local need.
The service operates across two integrated functions:
- Community Development: Focuses on building community capacity and resilience by supporting the development of sustainable local initiatives that promote health and wellbeing.
- Community Connectors: Offers short to medium-term, one-to-one support using a strength-based, person-centred approach. Individuals are supported to identify personal goals, reduce loneliness, and connect with community resources that enhance their independence and quality of life.
The team is multi-disciplinary and staff work together to ensure residents are empowered to live independently, make informed choices, and remain connected to their communities.
